We are always taking on new patients and our current Practice Area covers the following postcodes; L8, L15, L17 & L18. If you live close but not in one of these postcodes call to ask if your specific address is within our boundary or use this postcode checker.
There are 3 ways to register, chose the one that is best for you;
1. Online at www.campusdoctor.co.uk/seftonpark (quickest and most environmentally friendly)
2. Print, complete and return a Registration Form and Questionnaire (find our address on our Home Page)
3. Call us on 01512958700 and ask for the forms to be sent to your home address
To complete the registration process you will also need to visit our Nurse Pat for a New Patient Health Check, please call to book and arrange blood screening tests.
If you are currently on repeat medication and need a prescription before completing the registration process please call to discuss this.
Please keep us up to date with any changes to your details including, address, phone number and email. If you move out of our Practice Area we ask you to register with a different GP to ensure the best quality of care is available to you
Please register all babies as soon as possible after hospital discharge, you will then receive your babies 6 week check and first immunisation appointments.
At any point of the registration process proof of address and/or identification may be ask for.
