What happens after your registration form has been submitted


Additional Information: If we need more details, we’ll contact you using the information you provided. Your registration will not be finalised until we receive these details.

Confirmation: You’ll receive a text message once your registration is complete.

Booking Your New Patient Appointment: You’ll get a link to book your new patient appointment online. Alternatively, you can call us on 0151 295 8700 to book. This appointment is with a nurse or healthcare assistant. They will ask you some questions about your lifestyle and medical history and take your height, weight and blood pressure. 

Tell Us Anything Else We Should Know: If you have any additional information that could help us provide the best care (e.g., reasonable adjustments, opting in for cervical screening after a gender change, or opting out of the Summary Care Record), please let us know.

Online Services

Set up an online account to manage prescriptions, book appointments, view parts of your medical record, and receive messages from your GP practice (make sure NHS App notifications are switched on).

  • Download the NHS App from the App Store on your phone or visit NHS Web Services on a computer
  • Follow the instructions in the app or on the screen to set up your account

How to Request an Appointment

Online: Submit a request online Monday to Friday, 8:00am to 6:30pm. We'll review and respond by the next working day.
*You may not be able to submit a request online within the first 48 hours of registering. Please speak to our Reception Team instead.

By Phone: Call 0151 295 8700 Monday to Friday, 8:00am to 6:30pm. GP appointments are booked on a first-come, first-served basis each day.

Interpreter Needed? Please let us know in advance so we can arrange this for you.

 

 

 

Prescriptions
  • Book a GP appointment for your first prescription
  • Order repeat prescriptions via the NHS App, by emailing prescriptions.seftonpark@nhs.net or hand your request in at the reception desk in writing
  • Allow 48 hours for processing
  • Attend your medication reviews and complete any monitoring tests promptly so your repeat prescriptions can continue without interruption.

 

 

Test Results
  • We will only contact you if follow-up is needed
  • If your results are normal, we won't contact you unless the tests were part of an annual review
  • You can review your results anytime on the NHS App

 

 

Keep Your Details Up to Date

It's important to tell us about any changes to your contact or personal details. If we don't have the right details, you might miss important information about your care, or your information could go to the wrong person.

How to update your details:

Email changemydetails.seftonpark@nhs.net with:

  • Your full name
  • Your date of birth
  • Old details
  • New details

Alternatively, you can hand it in at the reception desk in writing.
